To Place a Prototype Order:

• In the U.S. and Canada:
  • Online: Order On-Line from Maxim Direct. When the system asks you to indicate "Stage of Production," choose "PROTOTYPE."

  • By e-mail: Download the Excel spreadsheet Order Form, fill out form, and indicate PROTOTYPE ORDER on the form. E-mail the completed form to:
